Tesamorelin: The Peptide that Targets Visceral Fat

Tesamorelin is a Growth Hormone-releasing peptide studied for its ability to reduce visceral fat. 

Visceral fat sits deeper than subcutaneous fat. It wraps around the liver, intestines, and other organs.

Visceral fat is linked to a higher risk of insulin resistance, heart disease, and metabolic syndrome, and it doesn’t always respond to diet and exercise.

What Is Tesamorelin?

Tesamorelin is a synthetic analog of Growth Hormone-Releasing Hormone (GHRH), a hormone your body produces naturally to signal the pituitary gland.

When Tesamorelin binds to GHRH receptors, it prompts the pituitary gland to release more Human Growth Hormone (HGH).

The liver then converts HGH into Insulin-like Growth Factor 1 (IGF-1), which carries out many of Growth Hormone’s downstream effects on metabolism and tissue.

This indirect approach is different from taking HGH directly. Tesamorelin amplifies the body’s own signal rather than replacing the hormone itself, which helps preserve HGH’s natural release pattern.

Tesamorelin is chemically related to Sermorelin, another Growth Hormone-Releasing Peptide, though Tesamorelin has been studied specifically for its effect on visceral fat.

Tesamorelin is typically given as a once-daily subcutaneous injection, usually in the abdomen. It often comes as a powder (lyophilized), which is reconstituted with sterile water before use.

How Does Tesamorelin Target Visceral Fat?

Visceral fat is more metabolically active than subcutaneous fat (the fat just under the skin). That means it functions more like an organ, releasing hormones, fatty acids, and other chemicals. It also has a deeper effect on negative health outcomes like insulin resistance and inflammation.

Growth Hormone and IGF-1 both play a role in lipolysis (fat burning), and research suggests visceral fat responds more readily to Growth Hormone signaling than fat elsewhere in the body. This is why Tesamorelin has shown such promising effects on visceral fat specifically.

Sermorelin also triggers hGH release, but it doesn’t reduce visceral fat in the same way. This is likely because Tesamorelin is more potent than Sermorelin and triggers more sustained hGH levels, which in turns boosts lipolysis.

What Does the Research Show About Tesamorelin and Visceral Fat?

In two 26-week clinical trials, adults treated with Tesamorelin saw visceral fat reductions of 14% to 18%, measured by imaging, compared with slight increases in the placebo group. 

Patients who continued treatment for a full year maintained those reductions, while patients who switched to a placebo saw their visceral fat begin to increase again. 

This pattern suggests a longer Tesamorelin protocol may support a shift in fat storage that lasts beyond the treatment.  

Visceral fat is difficult to reduce through lifestyle changes alone, which is why researchers have studied Tesamorelin for this purpose.  

Some patients in these studies also saw improvements in triglycerides and other metabolic markers associated with visceral fat. Tesamorelin isn’t typically used as a treatment for high cholesterol or triglycerides on its own, but this shows promising health benefits of a long-term protocol.

What Are the Potential Side Effects of Tesamorelin?

Like other Growth Hormone-Releasing Peptides, Tesamorelin carries some common, mild to moderate side effects.

  • Swelling in the hands or feet
  • Tingling or numbness in the extremities

These effects are generally described as mild in clinical studies and tend to improve as the body adjusts to treatment.

Tesamorelin isn’t appropriate for everyone. Tesamorelin is most often considered by patients focused on reducing visceral fat as part of broader body composition and longevity goals. It’s not typically used on its own for general weight loss, though it may be combined with GLP-1 medication as appropriate.
