What Is Total Testosterone?
Total Testosterone is the complete amount of Testosterone circulating in your bloodstream.
This includes both the Testosterone that’s available to your cells and the portion that’s bound to carrier proteins like Sex Hormone Binding Globulin (SHBG).
Most standard labs report total Testosterone in nanograms per deciliter (ng/dL). LabCorp’s reference range for adult men is 264 ng/dL to 916 ng/dL — a range wide enough that two men with very different symptom profiles can both fall inside it.
Total Testosterone tells you how much Testosterone is present in circulation. What it doesn’t tell you is how much your body can actually use.
What Is Free Testosterone?
Free Testosterone is the small fraction of total Testosterone that is not bound to any protein.
It’s typically around 1–4% of the total. Because it’s unbound, it can enter cells directly to be utilized.
This is the form that drives the characteristics most often associated with healthy Testosterone levels: energy, libido, muscle growth, mood stability, mental clarity, and more.

Bioavailable Testosterone includes free Testosterone plus Testosterone loosely bound to albumin — a blood protein that releases Testosterone readily when the body needs it.
In practice, bioavailable Testosterone gives providers a clearer view of how much Testosterone your body has real access to. It’s especially useful when SHBG is elevated or when symptoms don’t align with total Testosterone results.
What Is SHBG, and Why Does It Matter?
Sex Hormone Binding Globulin (SHBG) is a protein produced by the liver that binds tightly to Testosterone — so tightly that the Testosterone it holds becomes essentially inactive.
The higher your SHBG levels, the less total Testosterone you have available for use. This is why two men with identical total Testosterone levels can have very different symptoms.
SHBG tends to rise with age. It can also be elevated by liver changes, certain medications, high-estrogen states, and low thyroid function.
When SHBG is high, even a total Testosterone in the mid-normal range can leave a patient with functionally low hormone levels and deficiency symptoms. Total Testosterone, in that scenario, tells an incomplete story.
Which Testosterone Number Matters Most for Symptoms?
For most patients, free Testosterone is the most clinically useful number when it comes to explaining how you feel.
That said, free Testosterone, total Testosterone, and SHGB are all important pieces of the picture.
Total Testosterone is a starting point. Free Testosterone reflects what’s actually reaching your tissues. SHBG levels illustrate how Testosterone is being utilized.
What Do Optimal Testosterone Ranges Look Like?
Standard reference ranges are built from large population averages. They’re designed to screen for disease — not to optimize how you feel and function.
For total Testosterone, LabCorp’s reference range spans 264 ng/dL to 916 ng/dL. A man at 280 ng/dL is technically “normal”, but many patients at that level experience deficiency symptoms.
For free Testosterone, Labcorp has different reference ranges depending on age. For men aged 40-49, the range is 6.8-21.5 pg/dL. For men aged 50-59, the reference range is 7.2–24.0 pg/mL. The right target depends on the individual patient’s symptoms, age, and clinical context.
