What is HCG and why do some male TRT plans include it?

Human Chorionic Gonadotropin (HCG) is a hormone that mimics Luteinizing Hormone (LH). LH controls the signal that the brain sends to the testes to stimulate natural Testosterone production.

HCG is commonly prescribed alongside men’s Testosterone Replacement Therapy (TRT) for two primary reasons:

  • Fertility preservation
  • Testicular health

When the body receives external Testosterone through TRT, the brain reduces its own signaling to the testes. Over time, this can cause the testes to decrease in size and reduce sperm production. HCG steps in to maintain that natural signaling, helping preserve testicular function and sperm count during treatment.

Not every TRT patient requires HCG. Your provider will evaluate your lab results, health history, and personal goals — including whether fertility is a current or future consideration — to determine whether HCG belongs in your protocol.

If your circumstances change, HCG can be added to your treatment plan at a later point, as well.
