I need to refill my meds. What do I need to provide for my Primary Care appointment?

Defy Medical offers limited medication refill services through telemedicine Primary Care. We require pharmacy records from the past 12 months that prove you’ve been prescribed the medication(s).

When you fill out our secure online Primary Care form, you’ll be able to attach the required documents (up to 3) for review.

We do not prescribe controlled substances (even if you have been prescribed them before) through Primary Care.
