I have Hypertension. What do I need to provide for my Primary Care appointment?

We require prior medical or pharmacy records that indicate your treatment for hypertension. We also require a limited lab panel that includes:

• Comprehensive Metabolic Panel
• Lipid Panel
• TSH
• HbA1C

When you fill out our secure online Primary Care form, you’ll be able to attach the required documents (up to 3) for review. You can purchase lab work through Defy Medical, as well.
