You can also access information about your lab results, care plan, prescription orders, and more directly through your Patient Portal account at any time.

You have the right to inspect and receive copies of medical information we maintain, with certain limited exceptions. If we maintain your information electronically, you may request an electronic copy or direct us to send it to a person or entity you designate. We may charge an administrative fee for the labor involved in copying, mailing, or retrieving and providing electronic copies.
