How do I fill out an assessment in the app?

To fill out an assessment, click the clipboard icon at the bottom middle of the screen. This will take you to the Assessment page.

Click Continue to Assessment. You’ll go through a quick list of questions about your mood, libido, stress levels, and more. Rate your experience with these aspects from one to five stars.

Once you complete the daily assessment, you’ll see a page that averages your assessments over the last seven days, the last 30 days, and the last six months.

We encourage patients to submit daily assessments to keep track of key symptoms. Then you can discuss your assessment scores with your provider at your next consultation.
