How do I make a Patient Portal account?

Staff will create an account for you in the Patient Portal once you become a patient with Defy Medical.

To get started, you can call us at 813-445-7342 or fill out this web form.

Once a member of our staff creates an account for you, you’ll receive a password reset email to complete your account setup. Then you’ll be able to utilize the Patient Portal.
