What are pellets?

Hormone pellets are implantable capsules that contain testosterone or estradiol. These capsules are inserted under the skin in order to provide consistent and physiologic doses of these essential hormones. Our preferred FDA-registered compounding pharmacy is recognized throughout the industry for their outstanding quality of implantable, sterile pellets. These pellets are sterilized using gamma radiation and every batch is tested by a third-party analytical lab for sterility, potency, and endotoxins before being dispensed. They are also designed to be resistant to breakage and are capsule-shaped for easier insertion. All hormone pellets are manufactured with cGMP quality and compounded PVP (povidone) free for patients sensitive to these binders.

Where are hormone pellets inserted?

The insertion of pellets is a relatively simple in-office procedure done under local anesthesia and performed by a trained medical doctor or mid-level practitioner. The pellets are implanted in the subcutaneous tissue of the hip and buttock area under sterile conditions. By making a small incision, the pellet is inserted beneath the fatty lining of the skin. Once the pellet has been properly encapsulated in the body, it is then closed with surgical glue or sterile medical tape strips. Implants placed under the skin provide the consistent release of small, physiologic hormone doses that have been shown to provide the benefits listed above.

How long do the hormone pellets last?

Depending on the rate at which the hormones are metabolized, hormone pellet implants will typically last for about 3-4 months in women and 4-5 months in men. Patients usually experience symptom relief within the first 48 hours, although some take up to two weeks before noticing a difference. The pellets do not require removal as they are dissolved completely by the body.2

Are there side effects?

There are generally minimal side effects associated with pellet implant procedures. Complications are very rare, although side effects may include bleeding, bruising, infection, swelling, pain, lack of effect (typically a result from lack of absorption), sensitivity at the insertion site, and pellet extrusion.3,4

How do I get started?

Prior to commencing any form of HRT with our clinic, our physicians require that all patients have comprehensive hormone lab testing completed in order to better evaluate each patient’s overall health and individual hormone levels. This blood work is then assessed in conjunction with symptoms, medical history, and a recent physical examination to customize an individualized treatment plan for the patient based.

If pellets are the desired approach to HRT, the prescribed dosage and follow-up schedule will be determined during the initial evaluation with one of our practitioners. Thereafter, the pellet insertion procedure will be performed by a trained medical provider.

How are hormones monitored after insertion?

Follow-up blood testing, physician lab review, and pellet reinsertion are required every 3-4 months for all patients undergoing hormone pellet therapy. In some instances, the medical provider may want to see specific lab tests more frequently or request additional lab tests if there is a need for closer monitoring of certain symptoms. Each patient’s subjective response to pellet therapy may differ, therefore follow-up blood work may be required earlier than 3-4 months.

It’s important to stay compliant with blood work and pellet reinsertion so there is no lapse in treatment. We utilize an automated reminder system that will send a notification by phone and email to each patient notifying them of when they are due for follow-up lab work, consultation, and pellet reinsertion. This gives the patient an abundance of time to order lab work and have it completed.

What is PRP?

Platelet rich plasma (PRP) is a concentrated blood sample that contains high levels of platelets, a critical component of the wound healing process. Platelets contain growth factors that are responsible for stimulating tissue generation and repair. Growth factors derived from platelets are responsible for soft tissue repair, bone regeneration, development of new blood vessels, and stimulation of the wound healing process. Human blood contains mesenchymal stem cells and autologous blood products that contain essential and specific growth factors that assist in tissue regeneration and healing.

The PRP procedure involves drawing out one’s own blood and centrifuging it so that the plasma with platelets collects in the tube. This plasma rich in platelets and growth factors is very useful for tissue regeneration and healing.


Platelets mediate cell differentiation, migration, and cell-to-cell adhesion through the release of various growth factors, cytokines, and adhesion molecules. One of the most important functions of platelets is their role in promoting angiogenesis, as physicians recognize that re-establishing blood flow through angiogenesis is critical for healing.28 Recent studies indicate that the optimal platelet concentration to stimulate angiogenesis ranged from 1.5 million to 3.0 million platelets per microliter (1.5 – 3.0 x 106 platelets/µL).16

Compounding Pharmacies

Compounding pharmacies not only make (compound) drugs like testosterone, but they also compound nutritional supplement medications like injectable/IV nutrients, and even vitamin or amino acid capsules, powders, and tablets. The differences between nutritional supplements a compounding pharmacy makes and what you find over the counter at local supplement stores like the Vitamin Shoppe or GNC are:

  • Compounding pharmacies use only ingredients that come from FDA approved and regulated pharmaceutical (USP) sources while over-the-counter supplement companies use ingredients not approved by the government for safety and effectiveness before they are marketed.
  • All compounding pharmacies must follow strict state and federal guidelines when compounding any product, whether they make a medication or nutritional supplement.  All compounded products must be tested for sterility, potency, and toxins prior to leaving the pharmacy.
  • Compounding pharmacies only use ingredients in the purest form, free from dyes and fillers.
  • Over-the-counter supplements come in a one-size-fits all dose while nutritional supplements compounded at a pharmacy can be customized for strength, combinations with other nutrients, and delivery system- capsule, lozenge, injectable, etc.
  • Since anything coming from a compounding pharmacy requires a prescription, even a multi-vitamin, Defy Medical will generate the prescription for each order at no additional cost. No appointment is required when ordering nutritional supplements.

Compounding pharmacies also make cosmetic face creams, topical hair restoration, and nutraceuticals we commonly prescribe at Defy. In addition, many compounding pharmacies specialize in compounding important hard to get medications like certain cancer drugs, HIV medications, and others. What about Metabolic Maintenance (MM) and Orthomolecular (OM)?

MM and OM are Defy's preferred source for over the counter supplements not available at a compounding pharmacy.  We use these companies since they voluntarily and certifiably follow the same high standards for quality control and testing as required by a pharmacy.  They make high quality supplements specifically for doctor's practices using the same quality of ingredients as a pharmacy. Compounding pharmacies do not have the ability to make every nutritional supplement and/or combination that we may need, therefore MM and OM provide a great low cost source for an array of nutritional supplements that we can provide our patients.  In addition, cost for nutritional supplements can also be lower when purchasing through MM or OM.  Remember, when a product is compounded at a pharmacy there are additional costs especially since a licensed pharmacist must make it from scratch each time a prescription is received.  Compounding pharmacies should be used for nutritional supplements when the supplement is in a strength, combination, or delivery form not available anywhere else.  Ironically, sometimes items are cheaper at the compounding pharmacy than found at over the counter sources even MM and OM.  This confirms that you must also look at all sources when finding a particular product. PRP Therapy Risks

Because PRP effectively induces an inflammatory response, patients will most likely experience some post-procedural pain at the injection site for up to 48 hours. Higher volumes of injected PRP may cause more post-injection pain. Post-procedural pain can typically be managed with ice and over-the-counter pain relievers like acetaminophen (Tylenol®).

Non-steroidal anti-inflammatory agents (NSAIDS) such as aspirin (Bayer®) or ibuprofen (Motrin®, Advil®) should be avoided as they exhibit anti-platelet and anti-coagulant effects, which may diminish the effectiveness of the PRP treatment.

Purpose of Initial Labs

There is no blood test for the diagnosis of osteoarthritis. Blood tests are performed to exclude diseases that can cause secondary osteoarthritis , as well as to exclude other arthritis conditions that can mimic osteoarthritis. Separately, certain hormones can assist with the management of inflammation and pain perception. In addition, sex, growth, and thyroid hormones promote the restoration and strength of bone, muscle, and important supporting structures which may aid in the healing of certain injuries. Lab results will be discussed during the procedure and treatments may be offered if any deficiencies are diagnosed.

How many treatments are necessary?

Some patients with more difficult injuries may require more than one series of injections to achieve a successful outcome. The majority of PRP patients find that within 3 months of their first treatment they can return to most or all of the activities they were doing before the pain started. Each set of treatments is spaced approximately 12-24 weeks apart. There is no limit to the number of treatments you can have. The risks and side effects do not change with the number of injections. We have patients who have reported experiencing significant benefit after a single treatment, while some experienced an increased benefit with 2-3 treatments.
